I use either a five turn surgeons knot or a double uni knot. It has been posted about a zillion times.
I do not make up leaders ahead of time. I just carry some spools of straight mono in the tests that I like. It only takes a few minutes to add sections to a leader. I don't mind doing that. I have more trouble trying to locate premade leaders and untangling them and figuring out which ones are for what bright idea I had when making them up ahead of time. That can take a half hour. My fly box is often a paper bag with some flies stuck in it and if I am getting anal I may put that in a plastic food bag. The top of my waders above the belt is my tackle carrier.
I do carry a hook file. A light that can show me the bottom when I wade. I do not like those gadget lights.
You can make this complex and difficult or travel light. Making sure you have everything that you can possibly need will make you crazy over time and I think that is why so many people like to hang out in parking lots when they should be fishing. They do not want to leave anything behind and so they hang out and show stuff to anyone who will look.

A spool of mono, a paper bag and a fly rod.
Makes the fishing a lot easier and I leave all the "If Only's," behind. I am not into labor and time saving devices. I make do.

It works for me except when it doesn't - but there is tomorrow and I do not mind adjusting to each days twists and turns.
